Please remember to complete the home health screening form each day your child is scheduled to be in our school buildings.<\/p>\n
To best assist our buildings and to avoid reminder phone calls and emails, here are a few tips for success:<\/p>\n
Please complete the screening form on the day your child is scheduled to be in school<\/p>\n<\/li>\n
Complete the form before your child leaves for school<\/p>\n<\/li>\n
Enter your child\u2019s first name and last name exactly as it appears in PowerSchool<\/p>\n<\/li>\n
An email reminder is sent each day at approximately 5:45 am. If you are not receiving it, please contact the main office of your child\u2019s school.<\/p>\n<\/li>\n
